ANNUAL LONG TERM CONDITION REVIEWS

We recall patients with Long Term Conditions for an annual review in line with best practice and from 2018 we have started a programme whereby patients will be called once each year, however many conditions they have. this will minimise the number of visits and improve the coordinatuion of your care.

You may be invited for an appointment to check your blood pressure and perform some blood tests, or to see a member of our clinical team, depending on the conditions.

The following conditions are included in our annual recall:

BP + Pulse:	Hypertension and Atrial Fibrillation
Cardiovascular:	Coronary Heart Disease, Heart Failure, Peripheral arterial disease or TIA
Diabetes:	Type 1 and Type II Diabetes, Pre-Diabetes, Gestational Diabetes
Renal:	Chronic Kidney Disease
Mental Health:	Bipolar, Dementia, Learning Disability, Psychosis
Respiratory:	Asthma (if bronchodilator issued in last year), Chronic Obstructive Pulmonary Disease
Other:	Rheumatoid arthritis, Severe Frailty, Post-Bariatric Surgery
